Q. Why do I need to
provide a photo ID and my insurance card every time I come to the
hospital?
A. Lourdes needs to verify your personal and
insurance information each visit to protect your
identity. This is required and enforced by the Federal
Trade Commission. Your care will not be delayed due to lack of
identification.
Q. Do I need to
bring anything with me to register?
A. Yes, please bring a photo ID, insurance
card(s), and your physician's order, if applicable.
Q. Can I
pre-register for upcoming services?
A. Hospital Services
Patients scheduled for outpatient testing and surgical
procedures may pre-register by calling 607-584-5515, Monday through
Thursday from 8:00 AM to 7:00 PM, and Friday from 8:00 AM to 5:30
PM. Please have your insurance cards and health care provider's
written orders (if applicable) available when calling. Patients who
have pre-registered may go directly to their appointment location
upon arrival the day of the test or procedure. Please be sure to
check-in at your appointment location.
Physician Services
Patients scheduled for an appointment with a Lourdes provider
may pre-register by calling 607-584-5516, Monday through
Thursday from 8:00 AM to 7:00 PM, and Friday from 8:00 AM to 5:30
PM. Please have your insurance cards available when calling. Please
pre-register at least two (2) business days prior to your scheduled
appointment date. Be sure to check-in when you arrive for your
appointment.
