Full-time employees are defined as working 72-80
hours bi-weekly. Part-time employees are defined as working
30-71.99 hours bi-weekly.
Paid Time Off (PTO) - Accrual for time off for
vacation, sick, and personal days begins with date of hire.
Holiday - Time off for specific holidays are
designated in addition to PTO.
Bereavement Leave - Three days paid leave for
death in immediate family. Bereavement does not deplete PTO
Jury Duty - Regular full and part-time associates
will receive their normal daily pay for days scheduled to work but
serving Jury Duty. These days do not deplete PTO bank.
Health Insurance - Cost shared with Lourdes and
Dental Insurance - Three comprehensive plans are
offered; one with orthodontic benefits and the others with
different levels of reimbursement and no orthodontic benefit.
Vision Plan - Two comprehensive plans that
provide annual routine eye exams and contacts or frames and
Flexible Spending Accounts - Tax deferred
accounts for dependent care and unreimbursed medical (for
out-of-pocket health related expenses).
Life Insurance and Accidental Death &
Dismemberment - Lourdes provides associates with 1 x
annual salary in life insurance at no cost to the
Supplemental Life Insurance (Term and Whole Life)
- Associate, spouse, and child coverage available at group rates
and these benefits are portable.
Short-Term Disablitly - Twenty-six (26) weeks
maximum in fifty-two (52) week period; total of 60% of gross
earnings per week.
Long-Term Disability - Long-term disability
benefit for injuries or illnesses which exceed 180 days. The policy
replaces a maximum of 60% of gross earnings.
Supplemental Disability Buy-Up - (optional
through payroll deduction) - Opportunity to purchase an additional
10% benefit to replace a total of 70% of gross earnings while out
of work due to a non-work related injury or illness, for both
short-term and long-term disability.
Business Travel Accident - Lourdes provides this
benefit at no cost to associates. $300,000 benefit if associate
dies while on company business.
Prudential Long Tem Care Insurance - Provides a
daily benefit for long-term custodial care in a home setting or
skilled nursing facility.
403b - Plan allows tax deferred savings plus
hospital matching funds. Associates can contribute pay pre-tax and
Lourdes will match 50% of the contribution up to the first 6% of
pay. Associates are eligible to participate in the savings plan
immediately. Eligibility for the match is obtained after 1
year of service with 1,000+ hours. 100% immediate vesting for
Employer Automatic Contribution - Lourdes will
provide an employer automatic contribution if associate works at
least 1,000 hours in one calendar year. Thereafter, associate must
have a minimum of 500 hours per calendar year to receive the
Tuition Reimbursement - 100% of tuition
reimbursement for full-time and 75% for part-time up to maximum
annual benefit of $5,250.
Employee Assistance Program - Provides
confidential professional counseling services to associate and
members of household at no cost up to eight (8) sessions.
Referrals provided if additional assistance is needed.
Special Services - Free parking with 24-hour
security, Occupational Health (Employee Health) Office, Associate
Fitness Center, Elder Care Referral Service, On-Site Visions ATM,
direct deposit of paycheck to any authorized financial
Lourdes Family Care Pharmacy - Delivers to
offsite offices. Offers competitively priced over
the counter medications in addition to prescription
Gift Shop - Located in the atrium.
On-Site Day Care is provided by the
Kathryn & Nelson Ellis Child Care and Learning
Center, a fully-licensed child care center located on the
Lourdes Hospital campus. The Center is open year-round, Monday
through Friday, and provides care to children ages 6 weeks through
6 years of age for any Lourdes employee. At time there may be a
waiting list for enrollment.
