Appointments

Calls to schedule appointments should be made during office hours. Please call the office 24 hours in advance if you are unable to keep an appointment. Providers have a daily preset schedule, therefore, arriving late will delay other appointments and you may be asked to reschedule.  Missing appointments may result in dismissal from the practice.

Insurance Referrals

Managed care insurance plans usually require a referral from your primary care provider for appointments with specialists, emergency room visits, out of network physician visits, etc.  It is your responsibility to inform this office at least 48 hours in advance for the authorization.  Our office should be notified the next working day of any emergency visits.

Reminders

2. Co-payments are expected at the time of visit.

3. Information about financial assistance is available by calling (607) 798-5506 or 798-5229
