Appointments
Calls to schedule appointments should be made during office hours listed above. Sick appointments fill up quickly, so you should call early in the day. Please call the office 24 hours in advance if you are unable to keep an appointment.
Missing appointments may result in dismissal from practice. Providers have a daily preset schedule, therefore, arriving late will delay other appointments and you may be asked to reschedule. Walk-ins are discouraged and can result in a long wait because scheduled appointments will be seen first.
Sick Calls Outside of Office Hours
If our office is closed and you have an emergency, you can reach the on call provider by dialing the office number listed above. Your call will be returned as soon as possible, so please be sure your phone is available to receive this call. Calls for routine requests should be made during regular office hours.
Here are a few things to have before calling the office:
- Age and approximate weight of child
- Always take your child's temperature before calling, saying he or she "feels warm" is not enough
- A list of symptoms
- Names and dosages of any medications taken
- A list of any allergies to medications
- Name and telephone number of your pharmacy
- Please inform our staff if you are a member of a local HMO or managed care plan
If it is an emergency
We will refer you to Lourdes Emergency Department for an evaluation. Please identify yourself as a patient with the Lourdes DePaul Pediatrics. This helps to ensure our ability to maintain close contact with the emergency department staff.
Prescriptions and Forms
Please plan ahead and allow 7 days for Physical forms for day care, camp, school, etc. We request 48 hours to refill prescriptions. If your child is ill, diagnosing and prescribing over the phone is usually not in their best interest and we would encourage you to call and make an appointment. You can also request a prescription renewal online.
What To Bring To Your First Office Visit
If transferring from another office, bring your child's immunization record to the first visit. If you do not have one, you should contact your child's previous provider or their school for a copy. A parent or a legal guardian should accompany the child to this first visit. Court documentation should be presented at this visit if you are the legal guardian/representative. You should make arrangements with your child's previous doctor to have records transferred. You may also stop in our office during regular business hours to sign a release so that we may obtain your child's records from their previous doctor. This is necessary to provide the best health care for your child and should be done in advance so records are available at the time of the first visit.
Gathering this information together for a permanent record will assist us in determining the best health care for your child. The detailed medical history we keep will prove valuable throughout your child's youth and adult life.
Insurance Referrals
Managed Care insurance plans require a referral from your primary care provider for appointments with specialists, emergency room visits, out of network physician visits, etc. It is your responsibility to inform this office at least 48 hours in advance for the authorization. Our office should be notified the next working day of any emergency visits.
We look forward to meeting the health care needs of your children. Please feel free to communicate your concerns and questions to us.
