Hours of Operation: 8:00am - 5:00pm Monday - Friday.  Evening and weekend appointments are also available.   Appointments are required. 
Evenings/Weekends: An on-call physician may be reached by calling the office telephone number. The answering service will assist you. 
Convenient Location: Easy access from the Vestal Parkway. Plenty of parking with easy access to our office. Our facility is a street-level building which provides easy access to our patients in wheelchairs or with disabilities. 
In Case of Emergency: Go to the nearest hospital emergency department if you have chest pains, difficulty breathing, serious injury or bleeding, severe pain, or are pregnant and experiencing contractions.

Associates in Family Medicine (AFM) is a group of four family physicians and highly trained clerical and clinical office staff who provide a full range of services to our patients.  We treat patients of all ages and our practice is accepting new patients.  Same day sick calls are available for established patients. 

Lab Services

Lab services are available 7:30am - 3:30pm Monday through Friday.  No appointment is necessary but please bring your physician order with you.

Prescription Refills 

All refills must be requested 24 hours in advance, Monday through Thursday. Refills will be called into the pharmacy late in the afternoon. You can also request a prescription renewal online.

Office Telephones 

Professional staff are available to take your calls during office hours.

Reminders 

1. Insurance cards must be presented at the time of visit. 

2. Co-payments are expected at the time of visit. 

3. Information about financial assistance is available by calling (607) 798-5506 or 798-5229

Your appointment is important. If you cannot meet your appointment please call our office to reschedule at least 24 hours in advance of your appointment.
