.Full-time employees are defined as working 80 hours bi-weekly. Part-time employees are defined as working 24-79 hours bi-weekly. A waiting period may apply.

Paid Time Off (PTO) - Accrual for time off for vacation, holiday, sick and personal days begins with date of hire.
 
Bereavement Leave - Three (3) days paid leave for death in immediate family. Bereavement does not deplete PTO bank.
 
Jury Duty – Regular full and part-time associates will receive their normal daily pay for days scheduled to work but serving Jury Duty. These days do not deplete PTO bank.
 
Health Insurance - PPO Plan. Cost shared with Lourdes and associate.
 
Dental Insurance – Two comprehensive plans are offered; one with orthodontic benefits and the other with lower reimbursements and no orthodontic benefit.
 
Vision Plan – Plan provides annual routine eye exam and contacts or frames and lenses.
 
Cancer Protector Plan - Insurance policy to help with unexpected expenses if cancer occurs. Will bridge the cost of treatment and actual payment of health insurance. Policy contains a wellness benefit.
 
Flexible Spending Accounts - Tax deferred accounts for dependent care (shelter up to $5000/yr), unreimbursed medical (up to $2500/yr for out-of-pocket health related expenses).
 
Life Insurance – Lourdes provides full-time associates with 1 x annual salary in life insurance at no cost to the associate.                                                
 
Supplemental Life Insurance (Term and Whole Life) - Associate, spouse and child coverage available at group rates and these benefits are portable.
 
Short Term Disablitly -  twenty-six (26) weeks maximum in fifty-two (52) week period; total of 50% of gross earnings up to a maximum of $1,500 per week.
 
Supplemental Disability Buy-Up – (optional through payroll deduction) – opportunity to purchase an additional 20% benefit to replace a total of 70% of gross earnings while out of work due to a non-work related injury or illness. Maximum total benefit $1,500 per week.
 
Long Term Disability – (optional through payroll deduction) – opportunity to purchase long-term disability benefit for injuries or illnesses which exceed 180 days. The policy replaces a maximum of 60% of gross earnings.  
 
Business Travel Accident – Lourdes provides this benefit at no cost to associates. $100,000 benefit if associate dies while on company business.
 
Aetna Long Tem Care Insurance - Provides a daily benefit for long-term custodial care in a home setting or skilled nursing facility.
 
403b - Plan allows tax deferred savings plus hospital matching funds. Associates can contribute up to $15,500 of pay pre-tax annually and Lourdes will match 50% of the contribution up to the first 5% of pay. Associates are eligible to participate in the savings plan immediately.  Eligibility for the match is obtained after 1 year of service with 1,000+ hours. 100% immediate vesting for employer match.
 
Employer Automatic Contribution – Lourdes will provide an employer automatic contribution if associate works at least 1,000 hours in one calendar year. Thereafter, associate must have a minimum of 500 hours per calendar year to receive the contribution.

  • Less than 5 years of service will receive 3% annual contribution 
  • 5 to 10 years of service will receive 3.5% annual contribution
  • 10 + years of service will receive  4% annual contribution

5 years of vesting service (1,000) are required.  Partial years do not count for vesting.
 
Tuition Reimbursement - 100% of tuition reimbursement for full-time and 75% for part-time up to maximum annual benefit of $5,250.
 
Employee Assistance Program - provides confidential professional counseling services to associate and members of household at no cost up to eight (8) sessions.  Referrals provided if additional assistance is needed.
 
Liberty Mutual Home/Auto Insurance - Payroll deducted; discount on premiums.
 
Special Services - free parking with 24-hour security, Employee Health Office, Associate Fitness Center, Elder Care Referral Service, On-Site ATMs for Visions, M&T and Horizons, direct deposit of paycheck to any authorized financial institution.
 
Lourdes Pharmacy - Payroll deduction is available.
 
Gift Shops – Payroll deduction is available for purchases through the gift shops in the Atrium and the DePaul building.
 
ConSern Loans for Education - Low cost educational loans for associate, spouse, family members.

On-Site Day Care is provided by the Kathryn & Nelson Ellis Child Care and Learning Center, a fully-licensed child care center located on the Lourdes Hospital campus. The Center is open year-round, Monday through Friday, and provides care to children ages 6 weeks through 6 years of age for any Lourdes employee. At time there may be a waiting list for enrollment.
